background/employment history. I further authorize the companies, agencies, or persons named below
to give any information regarding my employment, character and qualifications, together with any
information they may have regarding me whether or not it is in their records. I hereby release said
companies, agencies, or persons from all liability for any damage for issuing this information. A
photocopy or facsimile of this authorization shall be as valid as the original.
Proof of citizenship or immigration status will be required upon employment in accordance with the
Employment in Eligibility Verification (Form I-9).

Homes to obtain a consumer report and/or an investigative consumer report which may include the

3. (For truck drivers only) In accordance with the Department of Transportation Motor Carrier Safety

I understand that an “investigative consumer report” may include information as to my character, general
reputation, personal characteristics, and mode of living, which may be obtained by interviews with
individuals with whom I am acquainted or who may have knowledge concerning any such items of

I herby release and hold harmless any person, firm, or entity that discloses matters in accordance with this
authorization, as well as ELM Homes from liability that might otherwise result from the request for use of an/or
disclosure of any or all of the foregoing information.
I understand and acknowledge that under provision of the Fair Credit Reporting Act I may request a copy of any
consumer report from the consumer-reporting agency that compiled the report, after I have provided proper

In consideration of my employment, I agree to conform to the rules and regulations of ELM Homes and
my employment and compensation can be terminated, with or without cause, and with or without notice,
at any time, at the option of either the Company or myself. I understand that this application was
completed by me, and that all entries in it and information in it are true, correct and complete to the best
of my knowledge.
I further understand that this application is not, and is not intended to be a contract of employment, nor
does this application obligate the employer in any way if the employer decides to employ me. I
understand and agree that my employment is at-will and can be terminated by either party with or with
out notice, at any time, for any reason or no reason. No one other than an officer of ELM Homes has any
authority to enter into any agreement for employment for any specified period of time or to make any
agreement contrary to the foregoing and then only in writing signed by the officer.

As part of our hiring background and investigation, we may obtain consumer reports to prepare an investigative consumer report. The investigative consumer reports may consist of contacting all listed prior employers to verify your employment history. It may also include, but not be limited to criminal history reports and driving history records. Under the provisions of the Fair Credit Reporting Act (15 USC at 1681-1681u) as amended, before we can seek such reports, we must have your written permission to obtain the information. You have the right, upon written request, to a complete and accurate disclosure of the nature and scope of the investigation.
